Emergency Information Poster - Must be completed and affixed to the entrances to all wet labs. A copy is sent to Public Safety.

Hazardous Materials Shipping - Complete the on-line form to request assistance with shipping hazardous materials.

Laboratory Safety Self-Audit Checklist - Useful in conducting laboratory inspections.

Laser Safety Self-Audit Checklist - Useful in conducting inspections of laser systems.

Parental Consent Form - Required for approval of a 16 or 17 year old working (paid or unpaid) in a laboratory. See High School Students in the Lab for more information.

Particularly Hazardous Substance Approval Form - Must be completed and sent to Principal Investigator for approval to work with Particularly Hazardous Substances.

Proposal for Minor Working in a Lab - Required for approval of a 16 or 17 year old working (paid or unpaid) in a laboratory. See High School Students in the Lab for more information.

Report of Possible Chemical Overexposure - Must be completed and sent to Chemical Hygiene Officer and EHS in the event of a potential overexposure to a chemical.

Waste Inspection Checklist - Helpful in conducting inspections of chemical waste collection and storage.

Facility Modification Form - SEAS only - Must be completed and submitted on-line to request changes in building infrastructure or utilities to support planned research.
