3. Where is the Chemical Hygiene Plan?  What information is in it?

4. What do you do in the event of a fire? (including evacuation routes)

5. Where are the nearest eyewash and safety showers?  How do you operate them?

6. How do you know that your fume hood is operating effectively?

7. When should you wear gloves?  Eye protection?  How do you select the appropriate gloves?

8. Where do you keep your respirator?

9. Where can you find health and safety information, including MSDSs?

10. Do you use carcinogens, reproductive toxins or highly toxic agents?  If yes, have you received Prior Approval for use of these agents?

11. What are the most hazardous materials you use?  What are the hazards?  What precautions must  you take?

12. How do you clean up a spill of this material?

13. How do you dispose of the waste?
