Report Structure

Table of Contents

1.0  Executive Summary

Includes general observations, best practices and a summary of all recommendations, suitable for posting or distribution.  The recommendations are given in tabular form, as follows:

  • Column 1: brief statement of the recommendation
  • Column 2: indication of which department holds primary responsibility of the action item.  Where EHS is indicated, the name of the contact person is also noted.
  • Column 3: section of the report with more details about the recommendation

2.0  Introduction

Provides background information about the review process and the report structure.

3.0  Review Protocol

Gives an overview of the protocol used during the review.  Describes each step of the process including:

3.1 Department Profile
3.2 Pre-Review Meeting
3.3 Site Visit
3.4 Follow-up

4.0  Overview of Departmental Safety Program

Details the safety program structure in the department.  Includes most of the information gathered during the site visit meeting, with a subsection for training issues and practices.

5.0  Health and Safety Program Review

Provides a detailed review of each program identified in the Department Profile.  Includes a subsection for each program, detailing observations and recommendations.

Appendix A: Department Profile

A copy of the completed departmental profile, identifying which programs apply within the department.

Appendix B: Notification Memos to Departmental Personnel

Copies of correspondence from the department notifying personnel about the review.

Appendix C: Records Review Checklist

The checklist used by EHS detailing the recordkeeping requirements for each applicable safety program and indicating whether or not the records were in place.

Appendix D: Questions for Site Visit Meeting

The list of questions asked by EHS during the opening meeting of the site visit.

Appendix E: Questions for Individuals During Walkaround Survey

The list of questions addressed to laboratory workers by EHS during the walkaround portion of the site visit.

Appendix F: Environmental Health and Safety Contact Information

Provides the names, e-mail addresses and phone numbers for members of the EHS review team and any other individuals given responsibility for recommendations.
